May 28th - Registrations must be complete.

June 4th - Final payments due.

You can make payments here or in the office. $50 deposit due April 16th. No sign ups after May 21st.

All students are required to have a physical signed by a doctor after June 12, 2021. Paper copies of these must be turned in at one of the mandatory camp meetings.

Optional Forms

This form is for your convenience. Any sports physical form signed by a doctor after August 1, 2021 will work.

If your child takes medication, we MUST have this form completed and meds sent in original bottles with labels matching the names on this form.
